How did we do?

For anyone who has booked with us — a session, party, care home or school visit, SEND session or ongoing provision

Every question is optional · You can stay anonymous (you choose what we share) · Honest is best — the “could be better” bits help us most

Not for anything urgent. Worried about a child’s safety? Ring Ciera on 07748 656269 or Surrey C-SPA on 0300 470 9100. In an emergency, call 999.

2. About your visit

Who are you writing about?

What was your booking?

3. What was enjoyed

Kids’ corner

For the person who came to have a go. Drawing your favourite animal works best on paper — bring it along next time and we would love to see it.

How did you feel?

4. Anything you’ve noticed since?

Only if you have — “too early to say” is fine.

5. What could be better

Nothing is taken personally. If it is something you wish we offered rather than something that went wrong, our suggestions form is the better place for it.

Would you book with us again?

6. Only for EHCP-funded or commissioned provision

Skip if it doesn’t apply.

7. Can we share this?

Saying no to everything is fine. It is used only as you tick, and deleted if you ask.
